Filing a Report Online
De Anza College has adopted the online case-tracking and reporting system called Maxient, replacing the Advocate system that was used previously. The new system will provide a record of incidents and situations that the Student Judicial Affairs team can use to follow up and resolve cases effectively.
Maxient provides separate online forms for different types of reports or requests. Click each of the following for more information and a link to the appropriate form:
- Student code of conduct violations
- Sexual harassment, violence or misconduct – including relationship violence, threats or stalking
- Worrisome or threatening behaviors
Student Code of Conduct Violations
These include academic integrity and nonacademic violations such as misuse of alcohol or drugs, disorderly conduct, hazing, harassment, physical violence and other violations of the policies or rules of De Anza College or the Foothill-De Anza Community College District.
Sexual Harassment, Violence or Misconduct
– Including Relationship Violence, Threats or Stalking
This category includes dating violence, domestic violence, sexual harassment, sexual misconduct or stalking behaviors. The college Title IX coordinator works with the Student Judicial Affairs team to address these concerns.
