- Read and re-read this document (above) prior to entering Q4. It contains instructions about your orientation.
- Read and follow instructions for submitting onboarding paperwork for the clinical site (below).
- You will need to submit a Quantiferon Gold test as directed by Lead Instructor. If
you have had a positive QFT and have submitted a chest Xray instead, please submit
an annual surveillance form (available at Campus Health) by the due date. Chest Xrays cannot be accepted without documentation of a positive QFT in your file.
- For more information about all required health documentation, including chest Xrays, see your Student Handbook.
- Do your 2nd year background check/drug screening as directed by Lead Instructor. See the Nursing Student Handbook for information about this. Do NOT do this earlier than instructed by Lead instructor! Initiating this process prior to the assigned time frame means it will need to be repeated, at additional cost to you.
- For each quarter, a COVID test must be done within 30 days of the start of school. Bring a copy of a negative result to Orientation day. This guideline may change at any time.
- If it is Fall quarter, please bring a copy of your annual flu shot to your orientation day. If the flu shot is not yet available, get your shot immediately as it becomes available, and bring the documentation to clinical promptly.
- EMAIL a copy of ALL health and/or CPR updates to the Nursing Office, specifically for the Nursing Department record. This includes the QFT results and Flu Shot documentation. Do not submit Covid or background/drug testing results. Note: Instructors will not submit your documents to the Nursing Office on your behalf.

link above. PRINT your name legibly on EACH page. These include ALL pages of the Life Safety checklist, the LAST page only of the Confidentiality Statement, the Excellence Standards page, and the
Corporate Compliance form. For "Department," write "DAC." You must BRING these to
your first ECH orientation day. Do NOT include copies of any immunizations, CPR, background check or drug testing
documentation with your documentation (DO submit any/all health/CPR updates to the Nursing Office by the due date in Student Handbook though!!).
- Life Safety checklist - check each box separately (do not draw one line down the page through all the boxes). PRINT name clearly!
- Confidentiality/Privacy statement - read all pages, sign and bring last page only. PRINT name clearly at top of page.
- Excellence Standards. PRINT name clearly at top of page.

- Covid documentation - bring a copy to Orientation Day 1 with you. It will be retained
